Phase 1:
Decrease Risk Day 1
Goal: You’ll know your personal risks & how to address them

Timeframe: 1 Week (1 - 3 Visits)*

Establish family & client goals

Home Fall Risk review and recommendations

Strength and balance assessment

Start caregiver training to promote independence

Adaptive equipment inspection and recommendations

Medication review to identify possible fall risk side effects


Phase 2:
Strength and Stability
Goal: You’ll feel strong & energized to take on the day

Timeframe: 7 - 10 Weeks (2 - 3 Visits per week)*

Custom strength & balance movements

Track & measure progress

Daily movement activities

Guided training to make everyday activities easier
Phase 3:
Confidence and Beyond
Goal: Feel confident without fear of losing independence

Timeframe: 1 Week (1 - 2 Visits)*

What’s included:

Identify top 2 weakness areas, if any

Create plan for safe habits & routines
-
Caregiver/family training
-
General nutrition review for long-term wellness
